We are offering a $500 sign-on bonus!!Our SeaView Adult Day Health Program is seeking a part-time (21 hours per week) Social Worker to provide case management services and monitor the mental health status of program participants.
The hours are a flexible day shift Monday through Friday, between the hours of 10:
00 am - 2:
15 pm Monday - Thursday and 10 am - 2:
00 pm Friday.
This position will be located in Mashpee, MA at our Seaview Adult Day Health Center.
Duties and
Responsibilities:
Provide case management servicesDevelop and lead groups for the participants.
Assist the program coordinator in the referral, admission, and discharge process.
Write progress notes quarterly.
Inform participants and their families/caregivers of available community services, and make referrals as indications warrant.
Attend monthly supervisor meetings with the Program Coordinator (Bi-weekly with LICSW if not licensed).
Assist, as necessary, in the delivery of other required program servicesParticipate in the bi-annual or quarterly review of each participant's care plan as well as assisting with the development of care plans.
Assist the Program Coordinator with the implementation of policies and procedures.
Assist the Program Coordinator with record reviews, documentation, compliance, and implementation of policy and procedure.
Ability to provide services outlined in this job description at any of the three Adult Day Health sites as assigned by the Program Coordinator.
Respond to reasonable expectations of the Agency and/or supervisor.
Education and Experience Requirements:
Bachelor's Degree required.
At least One (1) year of recent social work experience working with disabled/elderly adults in a professional capacity is required.
